  • What is a Patient-Centered Medical Home?

    You and your health care are at the center of our medical home team. A patient-centered medical home is health care focused on you, the patient. It is a partnership between you and your medical home team. 


    This team is made up of healthcare professionals committed to improving your overall health and to helping you reach your health goals. Instead of just being treated for a problem here and there without making a connection between symptoms, the patient-centered medical home focuses on connecting the dots and coordinating care. 


    Your health team may consist of your primary care doctor, nurses, specialty physicians, pharmacists, care coordinators and others depending on your needs. Dr. Munoz and Holistic Pediatrics Group, Inc. has been providing comprehensive primary care services to children who reside in Tampa for over 17 years. We are committed to providing quality health care with compassion,respect and excellence to all our patients. 

  • What can a Patient-Centered Medical Home do for me?
    • Assign a personal physician/provider to lead your healthcare team.
    • Provide you with better, more personalized and continuous care, because your healthcare team knows you and your health history.
    • Provide you with effective and quality care, using evidence-based medicine and clinical decision support tools to guide the treatment.
    • Guide you through the complex health care system to help you get the care you need either from us or others.
    • Offer you better access to care that is coordinated between your doctor, hospitals and specialists.
  • How can we work as a team to improve your Patient Centered Medical Home experience?

    Your healthcare team will partner with you to live healthier continuously rather than only reacting to illness and/or injury. A medical home increases your access to care and coordinates your care within our office, hospitals and your specialists.


    Medical Home's Responsibility


    • Provide a safe environment to talk about confidential
    • concerns.
    • Offer responses to ALL your questions and concerns at
    • each appointment.
    • Work with you in partnership, using shared decision-
    • making, to manage your health.
    • Serve as the leader of your healthcare team, coordinating
    • care both inside and outside the practice.
    • Inform you of the services offered within the practice.
    • Provide same day appointments with your physician or
    • member of your care team.

    Patient's Responsibility


    We want to help you take responsibility for your health. 


    Here are some things you can do:


    • Select the provider of your choice.
    • Keep all your scheduled appointments. If you are unable to keep an appointment, please give at least 24 hours notice of cancellation. 
    • Inform the doctor at every visit about any other visits you've had and any tests ordered by any other physician.
    • Take an active part in your health care. Think about changes that are happening.
    • Talk with your medical home team, friends and family about how they can help you make necessary changes so you can enjoy better health.
    • Write down and bring your questions and concerns to your appointment.
    • Make sure you know how to take your medicines. Bring all your medicines with you to your appointment and discuss with your doctor.
    • Talk to your doctor about all the medicines you are taking, even those given by a different doctor. Ask the doctor for help if you don't understand.
    • Don't stop taking prescription medicine without first talking with the doctor.
    • Offer any feedback you might have to help us improve your care.

Our Medical Home approach is patient-centered, collaborative, coordinated, accessible, and team-based. We work to ensure that your child's physical and mental health needs are met, whether acute or chronic. By utilizing dedicated nursing staff to assist with referrals, prior authorizations, special care needs, and follow ups, we are able to streamline communication and care between our providers both inside and outside of our practice.
